i have 75,000 mile on my 04.5 3500 2wd with a NV5600, just last week i installed a juice/attitude and driving with the power setting passed 2 i can feel a jerky/tugging feeling when i get into the throttle,i have never experinced a slipping clutch before, but i think it is about to go out.
i hope its the clutch and not the transmission, the oil needs to be changed but that should'nt be the problem.

my end goal is to get into the 500-550 hp range
what brand and model clutch would be a good replacement

Call 1-800-988-4345 and ask for Peter. He'll ask some basic questions about your truck and what you use it for or have planned for it and will set you up with the proper clutch for your application.

Im running a south bend dual disc clutch, i love it. id reccomend a 3850 or something close, peter can tell you more, he picked out mine. also, something to ask about is the upgrade input and main shaft kits. the 5spds like ours have a history of the retaining nut on 5th gear backing off and being loose, theres kits out there to fix it but ask peter first. if is jerking in 5th, then that is the likely cause

He said its a NV5600 which is a six-speed and doesn't have the fifth gear nut problem. I'd third for the South Bend. I've been researching clutches a lot as of late and South Bend seems to be the best. However call and talk to them. They will steer you in the best direction. I hear South Bend has excellent customer service as well.

 One thing though, Do you have the NV5600 or the G56? On the NV5600 Reverse is to the Right and up the G56 is left and down. The reason I ask is because I thought they quit putting the New Venture unit in Dodges in 04 and the 04.5< had the G56. Though I've been known to be wrong before.

I beleive the NV-5600 was used untill partway through the 2005 model year, about when the Cummins "610" was introduced

Mine is an 05 and it has the nv 5600, reverse up and to the right. So it had to be a an 05.5 thing, I got the 600 not 610
